This online Complaint Form is offered to anyone who wishes to file a disability related complaint or request relating to programs, services, or activities, offered by the City of San Diego. All disability complaints or requests regarding access to City buildings, streets, or sidewalks may also be submitted in writing, by phone, or TTY.
