Fill out the online form below and press the "Submit" button at the end of the form. The completed application will then be sent to the Citywide Volunteer Coordinator via E-mail;
or,
Print the form, fill it out and mail it to the Citywide Volunteer Coordinator (at the address shown at the bottom of this page), or the appropriate Department Volunteer Manager on the Volunteer Opportunities page;
or,
Print the form, fill it out, and FAX it to the Citywide Volunteer Coordinator at (619) 424-0235 or to the appropriate Department Volunteer Manager if a FAX number is provided.
