PDF icon Form 501 (Candidate Intention Statement): A candidate for local office must file this form before they solicit or receive any contributions or before they make expenditures from personal funds on behalf of their candidacy. This original form is considered filed the date it is postmarked or hand delivered to the Office of the City Clerk (202 C Street, 2nd Floor, San Diego, CA 92101). Our office requires a wet signature and does not accept the form via e-mail.

PDF icon Form 410 (Statement of Organization): This form is to be filed within 10 days of a recipient committee or candidate receiving $2,000 in contributions. This original form must be mailed to the Secretary of State and a copy of this form must be filed with the Office of the City Clerk (202 C Street, 2nd Floor, San Diego, CA 92101).
