Forms & Permits

The San Diego Police Department has made available several forms for citizens to use to report specific crimes, request specific services, request a police ride-along and for a variety of other purposes.

Also available are many permits for specific business activities, police-regulated businesses, as well as alarm permit applications.

Any information you provide will be held in strict confidence by the San Diego Police Department, its agents, and employees. It will be used solely for the purpose of processing your requests. It will not be sold, distributed or otherwise disclosed for commercial purposes. This information may be released to third parties only with your prior written consent.

--ANNOUNCMENT--

The Police Regulated Occupation and Business fees are mandated under the San Diego Municipal Code Chapter 3, Article 3, Division 1. The San Diego Police Department proposed regulatory fees for Fiscal Year 2016 will be presented to the Budget Review Committee, which is open to the public, on May 4, 2015 at 9:00 a.m. in the 12th floor City Council Committee Room.  The City Administration Building is located at 202 C Street, San Diego, CA 92101.  If you would like to view this event, you can do so through the City’s live streaming broadcast at the following link:  http://www.sandiego.gov/

The FY2016 police regulated proposed user fees can be found by clicking on "Proposed User Fees 2016" (PDF).  Please refer to your particular regulated business/occupation to review the proposed fee adjustment.
