Eligible pregnant employees are entitled to Long-Term Disability (LTD) benefits if they have been medically certified as disabled from performing the duties of their regular occupation. In the majority of cases, the date of delivery is established as the date of disability. If secondary, complicating medical conditions arise during your pregnancy that disable you from working, medical certification describing your medical condition must be provided.

To apply for LTD benefits, submit the following forms to the LTD Office:

•  Long-Term Disability Application
• Attending Physician Statement  Note: If your health plan carrier is Kaiser, please contact the Kaiser Disability Office to request an Attending Physician Statement.

Following the birth of the child, please submit the following Certification of Birth form to the LTD Office:
